Information we collect

When you use this website, we may collect two kinds of information. First, information you choose to provide (such as your name, phone number, email address, and a description of your concern) when you submit a contact or appointment request form (processed through GoHighLevel) or complete new patient intake forms (processed through Jotform). Second, standard analytics data (such as pages visited, approximate location, device type, and how you found us) collected through Google Analytics 4 using cookies.

How we use it

We use the information you submit solely to respond to your inquiry, schedule and manage appointments, prepare for your visit, and communicate with you about your care. Analytics data is used in aggregate to understand how visitors use our site and to improve it. We do not sell, rent, or trade your personal information to third parties.

Third-party services

Form submissions are handled by GoHighLevel and Jotform, and analytics by Google, each under its own privacy policy. Embedded content (such as YouTube videos and review widgets) may set its own cookies when you interact with it. Health information you share with the clinic in the course of care is protected separately under our patient privacy practices and applicable law, including HIPAA.

Your choices

You may decline cookies in your browser settings, and you may contact us at any time to ask what information we hold about you or to request its correction or deletion, subject to legal record-keeping requirements.
